Releasor Release and Waiver of Liability Form
This Release and Waiver of Liability (“Release”) executed between May 22 to May 25, 2025 (date) by myself and/or dependents (“Releasor”) releases Iowa Asian Alliance (“Nonprofit”), a nonprofit organization organized and existing under the laws of the State of Iowa and each of its directors, officers, employees, and agents with regards to any potential liability arising from participation in the Asian Heritage Festival (“Festival”). The Releasor desires to provide services (“Services”) for Nonprofit to help Festival run smoothly and/or engage in activities, including, but not specifically limited to, takraw, ping-pong, any of the martial arts activities, and/or participating in any activity with the learning center, on the stage, or in the villages (“Activities”) occurring within Festival.
Releasor understands Releasor is responsible for his/her own insurance coverage in the event of bodily injury, personal injury, illness, death, or property damage/loss as a result of Releasor’s Services to Nonprofit or participation in Activities within Festival.
